Pathophysiology

At a biological level, this looks like a chronic inflammatory bowel disturbance with motility impairment. Diverticular disease can predispose to intermittent local inflammation, spasm, constipation, and gas retention, which explains rectal pain, cramping, bloating, and borborygmi.

The constant sore throat, white tongue, and fissuring/cuts of lips and tongue suggest persistent mucosal irritation. In practical terms, this may reflect dryness, reduced oral lubrication from poor sleep, mouth breathing, reduced intake, reflux/upper GI irritation, or a chronic inflammatory state.

The sleep fragmentation is important because it can amplify pain perception, reduce gut motility regulation, and increase morning symptoms. Waking every 45 minutes indicates the system is not entering sustained restorative sleep, which can then present as morning headache, fatigue, and heightened visceral sensitivity.

Most likely working condition:
Chronic symptomatic diverticular disease with constipation. The combination of long-standing diverticular history, intermittent constipation, bloating, abdominal cramps, and bowel noise is compatible with symptomatic diverticular disease. This is not sufficient to diagnose active diverticulitis.

Practical ranking of remedies
